Hair extensions are a collaborative investment. As part of your personalized experience, we ask that you complete the following agreement to help us understand your needs, inform you of best practices, and outline our shared responsibilities.

Please take a few moments to review each section carefully. Your comfort, satisfaction, and the integrity of your hair are our top priorities — and we’re honored to be part of your transformation.

Hair Extensions Commitment & Care Terms

PLEASE CHECK ALL BOXES.(Required)

1. A 50% non-refundable deposit is required to reserve a hair extension service appointment.

2. All custom-ordered hair extensions are final sale and non-refundable.

3. Changes or cancellations must be made at least 72 salon business hours in advance. Failure to do so will result in forfeiture of the deposit.

4. If I change the color or texture of my hair after the consultation but before installation, I must notify Parlour FL immediately. A new consultation and deposit may be required.

5. Parlour FL accepts payment by cash or credit card for hair extension services.

6. My stylist has explained the application method, hair quality, maintenance procedures, and any potential risks.

7. I agree to follow the home care instructions provided to maintain the quality and longevity of my extensions.

8. I have been informed of potential medical concerns or sensitivities related to hair extensions.

9. I understand Parlour FL and its stylists are not responsible for medical complications that may arise from wearing extensions.

10. I agree to attend a 2-week check-in and a 4-week maintenance or removal appointment as recommended.

11. Failure to attend or reschedule maintenance appointments within 72 hours may result in damage for which Parlour is not liable.

12. Parlour FL reserves the right to refuse service if my hair or scalp condition compromises installation quality or safety.

13. If damage occurs due to improper maintenance or alterations by myself or a third party, I understand that corrective work may be billed at $300+ per hour.

14. I understand that missed, late, or last-minute rescheduled appointments may result in a fee due to time reserved on my behalf.

15. If I change my hair color after booking but before installation, color-matching may not be possible.

16. If my hair is recolored after extension ordering, replacement or recoloring of extensions will be at my own expense.

17. I authorize Parlour FL to securely store my credit card and charge it in accordance with the cancellation, no-show, and maintenance policies. Stored card information will be kept securely and used only in accordance with these terms.

18. I understand that maintenance is required every 4–6 weeks and that failure to follow this schedule may result in damage or additional costs.

19. I will arrive to my appointment with clean, dry hair and acknowledge that appointments may take several hours.

20. I will not change my hair color prior to the scheduled installation unless I notify my stylist in advance.

21. Parlour FL may photograph or video document my hair transformation. If I do not consent, I must notify Parlour FL in writing prior to service.

22. I agree not to initiate chargebacks or payment disputes for services rendered in accordance with this agreement. I understand that doing so may result in legal action and recovery of incurred costs.

23. I understand that Parlour FL is not liable for damage to my extensions or natural hair caused by the use of unapproved styling tools, chemical products, helmets, hats, hairstyles, or accessories that may place tension or friction on the scalp or extensions.

24. I agree that any legal disputes arising from this agreement will be resolved through binding arbitration in the State of Florida, and not through litigation in a court of law.

- Failure on my part to follow home care or maintenance guidelines

- Improper handling of extensions after leaving the salon
- The use of third-party products or services not approved by Parlour FL

I acknowledge that, while Parlour FL follows the highest professional standards and takes every reasonable precaution, all hair extension services carry inherent risks. These may include, but are not limited to: scalp irritation, allergic reaction, discomfort, slippage, damage to natural hair, or tangling — particularly if care guidelines are not followed.

I hereby release, indemnify, and hold harmless Parlour NPB LLC, its stylists, employees, and affiliates from any and all liability, claims, damages, or expenses — including legal fees — that may arise directly or indirectly from my decision to receive hair extension services.

This waiver applies to all current and future extension services received at Parlour FL unless revoked in writing.

At-Home Maintenance Plan

To help maintain the longevity and beauty of your extensions, please follow these professional care guidelines:

Shampoo & Conditioning.(Required)

• Do not get hair wet or damp within 48 hours after installation. This includes sweating, sauna, or hot tubs.

• Detangle gently before wetting.

• Wash upright, not bent over.

• Shampoo in small circular motions; no back-and-forth rubbing.

• Avoid applying product to bonds/attachments.

• Rinse thoroughly.

• Use masques and conditioners only from mid-shaft to ends.

• Never rub hair with a towel.

• Dry attachments first, then the rest of hair.

• Always dry hair before bed.

• Use cool air after blow drying to smooth and seal.

• Never sleep with damp or wet hair.

Brushing and Styling(Required)

• Brush 2–3 times daily using an extension brush.

• Detangle ends first, then work upward.

• Section hair and gently brush near roots/attachments to prevent matting.

• Tie or braid hair before sleep, boating, swimming, windy days, etc.

• Sleeping with a silk bonnet in a low braid or bun is highly recommended to prevent tangling and friction.

• Always use a heat protectant.

• Avoid over-brushing or aggressive styling techniques.

• Avoid hairstyles that cause strain or tension such as high buns, tight ponytails, or slicked-back looks.

• Use caution with hats, helmets, or accessories that compress or rub against the bonds or attachments.

• Wrap hair in a silk scarf under helmets or hats for protection.

• Only use stylist-recommended or approved hair products.

• Do not apply oils or silicone-based products near bonds or attachments.

• Avoid high-protein or keratin-rich products unless instructed by your stylist — overuse can cause brittleness or tangling.

• Do not use purple/toning shampoos unless color-matched extensions were specifically blended to withstand them.

• Dampen hair and apply leave-in conditioner before swimming.

• Use only mineral-based sunscreen (avoid products with avobenzone or octocrylene).

• After water exposure, clarify and condition immediately.

• Use silk crunchies at the gym; avoid tight styles.

• Avoid tight helmets, hats, or high ponytails that cause tension on the rows.

• Wrap hair in a silk scarf under helmets or hats for protection from friction.

• After sweating, use a cool blow dryer to dry the roots and extensions.

• Never apply heat to wet extensions.
